Waiver and Release of Liability for Springfield Dance

Effective Date: 10/01/2024

Welcome to Springfield Dance (“we,” “us,” “our”), located at 5531 International Parkway, Springfield, Illinois. By participating in classes at Springfield Dance and/or using its facilities and services, located at 5531 International Parkway, Springfield, Illinois, I hereby agree, for myself and my family members, to the following terms:

1. I agree to abide by all posted rules and warnings, as well as any instructions provided by Studio staff.

2. I acknowledge the inherent risks associated with the activities offered by Studio and assume full responsibility for any personal injury or property damage incurred.

3. I agree to indemnify and defend Studio against any claims or expenses arising from my use of its facilities.

4. I agree to compensate Studio for any damages caused by my negligent, reckless, or willful actions.

5. I consent to my child's participation in Studio classes and agree to the terms on their behalf.

6. Any legal claim arising from participation in Studio activities shall be governed by Illinois law.This Privacy Policy explains how we collect, use, protect, and share information and data when you use our Website. By accessing and using our Website, you agree to the collection and use of information in accordance with this policy.

Payment Policy:

- All families must provide a Visa or Mastercard debit/credit card or bank account ACH for auto-pay. A 4% convenience fee applies to card payments.

- Payment can also be made upfront for the entire year by cash, check, or debit/credit card, or through alternative payment plans arranged with the studio office manager.

- A $40 registration fee is charged annually.

- Tuition fees are non-refundable, and no credits are issued for missed classes.

- Drops and class changes must be submitted in writing before the 1st of the month.

- In the event of natural disasters or closures, classes will shift to the Virtual Studio Experience until in-studio classes can resume.

Safety Spotters Agreement:

I authorize Springfield Dance, Inc. and its employees to use appropriate safety spotting maneuvers during Acrodance classes, provided that both parties are wearing appropriate personal protective equipment.

Photography Release:

I grant Springfield Dance, Inc. permission to use photographs or videos of myself or my child for promotional purposes, including studio publications and online platforms. I waive any rights to royalties or compensation for the use of these materials and release Springfield Dance, Inc. from any related claims.
